On-line- Registration and Speaker Forms

Please fill out the form and print it before emailing the form back to the Water Information Coordination Program Office

• Type in the information onto the on-line form.
• Click the Submit button to email a copy of your reply back to the Water Information Coordination Program Office.
  Click the clear button if you want to correct an item(s) and start again. (This option will clear all information in the form.)
• Note: Click Submit button to obtain an email confirmation reply of the information you have just typed. The computer system will respond with a message that your information has been sent. We will receive a copy of your information as well.
  

PDF Forms

Please! Do not fill out the form until you have saved it to your home area first.
To save the form in your home area do the following:

• Use the pull down "File Menu"
• Scroll down to the "Save as" option and save the file back to your home area. A dialog box will appear for you to type in the directory and file name for the file to reside in your home area.
• Once in your home area
  
  • Fill the form out by hand
  • Use the fax form provided or
  • your own fax form to return the form to us.
  • You can also send it back to us by email.

Microsoft Word document (Print and FAX)

Please do not fill out the forms until you have saved them back to your home area first.
To save the form in your home area do the following:

• Use the pull down "File Menu"
• Scroll down and select the "Save as" option to save it back to your home area. A dialog box will appear. Type the file name and directory for the file to reside in your home area.
• Once in your home area -
  • Fill the form out by hand
  • Use the fax form provided or your own fax form to return the form(s) to us.
  • Another option is send it back to us by email.

FAX Cover document (Optional - Print and FAX)

This fax cover sheet is for your convenience to accompany the Microsoft Word document or PDF document form(s). Please also consider the following options: use your own fax form, Advisory Committee on Water Information on-line form, and/or the Microsoft or PDF form that can be emailed back to us.

Note: Please do not fill out the FAX form until you have saved it back to your home area first.

To save the form in your home area do the following:

• Use the pull down "File Menu"
• Scroll down to the "Save as" option and save the file back to your home area. A dialog box will appear for you to type in the directory and file name for the file to reside in your home area.
  
• Once in your home area -
  
  • Type the information into the form
  • Print the fax form out to use as a cover for the registration or speaker form to fax back to the Water Information Coordination Program Office.
  • Suggestion: Save the form with the typed information also in your home area for reference purposes.
